Motivational Survey

Please take a few moments to complete the following survey. It will be used to determine, in your opinion, what is important to you about work, and how your company or department is doing at providing them.
This survey will be used for a classroom assignment and the information you provide is completely confidential.

In the first column, rank the 15 factors in order of importance to you personally. Rank the factors by placing a "1" by the most important item to you, a "2" by the next most important, and so on until you have ranked all factors. (15 will be the least important to you.)
In the second column, grade your company or department using an "A to F" grade, as defined below, as to how well in your opinion you are being provided these factors.
